Corrective action: Observed an employee start their shift without a hand wash prior to donning gloves. Education offered on when to wash requirements with staff member and person in charge.
None — 16: 4-501.114(A)-(E),(F)(1)-(2) Quaternary ammonia sanitizing solution intended to sanitize food contact surface(s) was not in the concentration range specified in the manufacture's required concentration. Observed sanitizer solutions testing at 0 ppm being dispensed from the three-compartment sink. Sanitizers were made manually and tested within approved concentration and person in charge will be placing a work order to maintenance the sanitizer dispenser to ensure adequate concentration is dispensed.
3-501.16 — (A)(2) Food item(s) that require temperature control for safety were cold-holding at internal temperatures greater than 41°F.
Corrective action: Observed TCS foods in the salad prep table holding between 46-48°F and sliced ham in the pizza prep table holding at 45°F. Coolers were kept closed during inspection to allow for temperature decline.
