Corrective action: Observed a few dead cockroaches under the water heater in the back of the kitchen and under the food prep table. Spoke with person in charge about removing pests to prevent the attraction of new pests. Outer openings were not protected against the entry of pests. Observed visible light appearing through the back door of the kitchen. Spoke with person in charge about a weather stripping to ensure all door cracks are sealed and protected against the entry of pests.

Corrective action: Observed a few boxes of food items being stored on the floor of the walk in freezer. Spoke with person in charge about ensuring food items are kept at least 6 inches above the ground to prevent the risk on contamination.

Spoke with person in charge about ensuring items are fixed. Per person in charge work orders have been put in for all items.

Corrective action: Observed an excessive buildup of grease along the fryers and under the equipment in the kitchen area. Spoke with person in charge about cleaning frequency for this item. Per person in charge item will be cleaned on the weekends.

Corrective action: Inspector observed the handwashing sink in the food prep area holding a maximum temperature of 75 degrees F. Person in charge will place a workorder to have the sink maintenanced.
None — 15: 3-302.11(A)(1)-(2) Raw animal foods were not separated from ready-to-eat foods during storage, or preparation, to prevent cross-contamination. Inspector observed a carton of unpasteurized eggs and a carton of unpasteurized liquid egg mix stored with RTE cut fruits. Person in charge moved the eggs to the bottom shelf during the inspection.

Corrective action: Inspector observed one carton of heavy cream without a datemark in the bistro area. Person in charge added a datemark during the inspection. 2) Refrigerated, ready-to-eat food(s) that require time or temperature control for safety was not discarded within 7 days of preparation or opening of product. Inspector observed one tote of shredded mozzarella cheese datemarked for 03/13. Person in charge disposed of the cheese during the inspection. See Voluntary Condemnation.

Corrective action: Inspector observed a damaged threshold on the back door. Person in charge will place a workorder to have the threshold maintenanced. 2) Dead or trapped birds, insects, rodents or other pests were not removed from the establishment often enough to prevent accumulation, decomposition, or the attraction of pests. Inspector observed dead cockroaches near the back door, dishwasher and bistro. Person in charge was instructed to continuously clean up the dead cockroaches as they appear.

Corrective action: Inspector observed excessive grease build up between the fryer and the flat top in the food prep area, as well as syrup build up in the dry storage area. Person in charge was instructed to perform a deep clean of these areas.
