None — 16: 4-602.11(A)(1)-(5) Equipment, utensils, and/or food-contact surfaces used in contact with food items that require time/temperature control for safety are not being cleaned at least every 4 hours. Observed a thermometer being used that did not go through the proper cleaning and sanitizing procedure. Discussed with the person in charge about ensuring the thermometer gets cleaned at the end of their breakfast service.
3-501.16 — (A)(1) Hot-held foods were holding at less than 135°F.
Corrective action: Observed a small portion of eggs hot holding in a small tray between 120-130°F for less than four hours. Discussed with the person in charge about ensuring hot held items are maintained at 135°F or higher. Eggs were reheated during the inspection and the tray was set to a higher hot holding temperature.
3-304.14 — Wiping cloths were not held in a reasonably clean sanitizer solution of appropriate concentration between use.
Corrective action: Wiping cloths being used to wipe down the thermometer were not in a sanitizing solution. During the inspection, inspector provided instruction and discussed with the person in charge that it is best to prepare a small bucket with a sanitizing solution at the proper concentration to hold wiping cloths being used for food contact surface.
3-304.12 — In-use utensils were improperly stored (without the handle above the top of the food, not on a clean surface, not in running water, or not in water of at least 135°F).
Corrective action: Observed serving utensil for the biscuits sitting in the container of biscuits with the handle touching the food items. Reviewed utensil storage requirements with person in charge and began placing utensils in the hot holding equipment during inspection.

5-205.11 — Hand washing sinks are not accessible to food employees.
Corrective action: Observed a large cart blocking access to the handwashing sink in the main kitchen prep area. Discussed with the person in charge about ensuring that handwashing sinks are readily available for food employees to wash their hands. Cart was moved during the inspection.
None — 15: 3-302.11(A)(1)-(2) Raw animal foods were not separated from ready-to-eat foods during storage, or preparation, to prevent cross-contamination. Observed a raw unpasteurized eggs above ready-to-eat food items in the reach in cold holding unit. Discussed with the person in charge that raw eggs must be adequately separated from ready to eat food items, such as putting it on the bottom shelf below ready-to-eat food items. Eggs were moved to the lower shelf away from ready-to-eat food items.
3-501.16 — (A)(1) Hot-held foods were holding at less than 135°F.
Corrective action: Observed sausage gravy hot holding in a small crock pot around 121°F for less than four hours. Discussed with the person in charge about ensuring hot held items are maintained at 135°F or higher. Sausage gravy was properly reheated during the inspection and the crock pot was set to a higher temperature for proper hot holding temperatures during the inspection.
3-501.17 — Refrigerated, ready-to-eat food(s) that require time or temperature control for safety and is held for more than 24 hours was not properly date marked.
Corrective action: Observed 1) a small container of sausage gravy in the main reach in cold holding unit without a date mark that is held for more than 24 hours. 2) a small carafe of milk in the main breakfast line area without a date mark that is held for more than 24 hours. Discussed with the person in charge about ensuring labels are on these food items to prevent holding these items for longer than is safe. Labels will be added to opened products moving forward.
6-202.15 — Outer openings were not protected against the entry of pests.
Corrective action: Observed holes and bent edges in the screen window of the kitchen allowing for easy access for pests to enter the facility. Reviewed expectation that the screen shall be replaced as a method of pest control during inspection.
None — 39: 3.306.13(B)-(C) Adequate protections against bare hand contact were not provided. Observed unwrapped individual bagels on the self-serve breakfast area available to grab without a protection barrier such as tongs or deli tissue. Tongs were added to this serving area to allow for protection against consumer contamination of ready to eat foods during inspection.

Corrective action: Observed serving utensils for the sausage gravy, eggs, potatoes and sausage sitting on the counter outside of temperature control. Reviewed utensil storage requirements with person in charge and began placing utensils in the hot holding equipment during inspection.
None — 51: 5-205.15(B) The plumbing system was not maintained in good repair. Observed a large buildup of ice in the mini reach in cold holding unit on the main breakfast line. Discussed with the person in charge about increasing the cleaning frequency to reduce the risk of a possible source of cross-contamination. Ice will be cleaned out of the machine.
