Corrective action: Observed the kitchen hand sink soap dispenser empty. Dish soap was being used for hand washing during inspection and the hand soap dispenser was to be refilled following inspection.

Corrective action: Observed an opened bag of cooked sausage without a date mark of when opened. Sausage was discarded during inspection, see Voluntary Condemnation.

Corrective action: Observed the consumer advisory messaging not matching burger items directly with ** as referenced on menu. Educated consumer advisory code requirements during inspection.

Corrective action: Observed employee exit the kitchen to the exterior walk-in cooler and re-enter the kitchen without changing gloves or washing hands. Employee was educated on when to wash requirements during inspection.

Corrective action: Observed no paper towels at the kitchen hand sink. Paper towels were restocked during inspection.

Observed raw meat fluid drainage on a box of tortilla chips in the freezer that had product thawing. Tortilla chips were discarded during inspection.

Corrective action: Observed 1.) thawed items in the reach-in freezer that had been defrosted and holding between 43-55°F; 2.) cut tomatoes on the pizza prep table holding at 44°F; and 3.) lettuce, tomatoes, and milk on the salad prep table holding between 45-49°F. Food items out of temperature control were discarded during inspection, see Voluntary Condemnation. Raw meats that were within temperature requirements were moved to the walk-in cooler during inspection.

Corrective action: Observed an opened milk carton without a date mark of when opened. Milk was discarded during inspection, see Voluntary Condemnation.

Corrective action: Observed raw and cooked meats in the reach-in refrigerator that had been defrosted due to equipment no longer maintaining freezing temperatures. Food items outside of temperature control were discarded during inspection. Reviewed with person in charge requirements of cleaning and servicing equipment prior to restocking to ensure foods will be kept frozen as required.

Corrective action: Observed the back entry door to the kitchen left open without adequate screening to prevent pests from entering the facility. Reviewed requirements with person in charge as a method of pest prevention. Pests were present in the establishment. Observed flies present in the kitchen area. Reviewed pest prevention methods including keeping doors closed, pest control professionals, and setting traps in adequate areas.

Corrective action: Observed buildup of dust and debris on condensers of cold holding equipment. Reviewed cleaning procedures with person in charge.

3-101.11 — Inspector observed mold growth on sliced pepperjack cheese, sliced american cheese, and diced green chile in the cold holding unit across from the flat top stove.
Corrective action: Person in charge discarded the items during the inspection. See voluntary condemnation.

Corrective action: Inspector observed the pizza dough spreader with excess dough and oil not cleaned after use or every four hours. Person in charge was instructed to clean the equipment at a greater frequency.
3-501.16 — (A)(2) Food item(s) that require temperature control for safety were cold-holding at internal temperatures greater than 41°F.
Corrective action: Inspector observed several TCS foods holding between 56-61°F for longer than four hours in the cold holding units across from the hot food prep line. Person in charge discarded the items during the inspection. See voluntary condemnation.
3-501.18 — Refrigerated, ready-to-eat food(s) that require time or temperature control for safety was not discarded within 7 days of preparation or opening of product.
Corrective action: Inspector observed Bacon prepared on 06/12/2026, Deli ham prepared on 06/13/26, Chicken prepared on 05/09/26, and Salsa prepared on 06/15/2026 that had not been disposed of after 7 days. Person in charge discarded the items during the inspection. See voluntary condemnation.
4-301.11 — The equipment available is not capable of cooling, heating, or holding foods at the temperatures required by the code.
Corrective action: Inspector observed the cold holding units across from the hot food prep line holding the food inside at an ambient temperature of 56-61°F. Person in charge will submit a work order to have this equipment repaired.
6-202.15 — Inspector observed several areas of heavy mouse fecal deposits throughout the facility, as well as many live flies.
Corrective action: Inspector discussed the frequency of cleaning up the mouse droppings, sealing the outer openings, and keeping a more regular pest control service. Person in charge was instructed to deep clean the areas with mouse droppings present and to keep their screen closed to prevent the entry of pest.
3-303.12 — Inspector observed water from a faulty cold holding unit dripping on top of cups of cheese that were stored inside.
Corrective action: Person in charge discarded the cheese and will submit a work order to have the equipment repaired.
4-602.13 — Nonfood contact surfaces of equipment were not kept free of an accumulation of dust, dirt, food residue and other debris.
Corrective action: Inspector observed heavy dust accumulation on the fans, ceiling vents, and refrigeration equipment screens. Person in charge was instructed to clean this equipment at greater frequency to prevent the circulation of dust and debris in the kitchen, as well as increase the functionality of the cooling equipment.
